Another two Aussies leave aged care
In January, we were delighted to approve funding for two Home Soon applicants, enabling one young person to leave aged care, and another to exit hospital and move into supported accommodation.
Youngcare’s Home Soon Grants, proudly supported by Suncorp Insurance, provide funding of $5,000-$20,000 per grant to help young people transition back into their community. A total of $33,934 was approved for intensive transition care services, and to purchase a bed and recliner chair specifically designed for the needs of people with Huntington’s disease.
Home Soon Grants remain open until 1 June 2016 and we look forward to receiving further applications and continuing our efforts to help more young Aussies return home.
